The official probate forms index
Every form on this page is published by a court, and every one of them is free. Probatly does not sell forms and does not host copies — it links you to the court’s own hub and names the form numbers, so you can go straight to the right document and check that you have the current version.
Never pay a form seller for a probate form. The blank forms that open, run and close an estate are published free by the courts that require them — the same forms, word for word, that form-selling sites charge for. If a site asks you to pay for a blank court form, close the tab and use the official hub for your state below. What can legitimately cost money is advice about your estate, from a licensed attorney in your state — never the blank form itself.
The states differ sharply. Some — California, Ohio, North Carolina and Georgia among them — run one mandatory statewide set used in every county. Others split the job between state rules and county packets, and one state in this index publishes no statewide library at all. Each section below says which position its state is in, honestly.
California
Judicial Council DE-series forms (mandatory statewide)
California Courts Self-Help — Probate — the official source for this state’s forms, free from the court.
- DE-111Petition for Probate — the form that opens the case
- DE-121Notice of Petition to Administer Estate
- DE-140Order for Probate
- DE-147Duties and Liabilities of Personal Representative
- DE-150Letters — your proof of authority
- DE-160 / DE-161Inventory and Appraisal, and its attachment
California's forms are the gold standard: one mandatory statewide set, free from the courts. Some counties (for example Riverside) add a small number of local forms.

Texas
No statewide form set — county courts publish their own
TexasLawHelp.org — free legal information and the small-estate affidavit — the official source for this state’s forms, free from the court.
- County setMontgomery County Probate Court No. 1 publishes a small-estate affidavit, an inventory template, and Independent/Dependent Administration handbooks in English and Spanish
- County setGalveston County's court publishes a partial set — note there is no form for the application to probate a will; that document is drafted
Texas is honest-to-goodness county country: there is no comprehensive statewide set of probate forms. Your county probate court or county clerk is the source, and TexasLawHelp fills part of the gap for the small-estate affidavit.

Florida
Florida Probate Rules + county clerk packets (two layers)
Florida Courts Help — Probate resources — the official source for this state’s forms, free from the court.
Florida Probate Rules — Jan 1, 2026 edition (official PDF via The Florida Bar) — the statewide rules that prescribe what each Florida probate filing must contain.
- Rule 5.200Petition for Administration — the rule prescribing what the opening petition must contain
- Rule 5.240Notice of Administration
- Rules (full text)The complete Florida Probate Rules, Jan 1, 2026 edition, published by The Florida Bar — the forms index links the official PDF
Florida does not publish one unified official forms packet. The statewide Probate Rules prescribe the contents of each filing, and circuits and county clerks publish their own packets and judicial checklists (for example the 12th Circuit's formal-administration checklists). Everything is free and official — it just lives in two layers.

- RW-02Petition for Probate and Grant of Letters — the standard opening filing
- Orphans' Court setThe statewide forms are hosted by the AOPC; county Register of Wills sites link directly to them and add local supplements

Illinois
Statewide standardised forms hub (IL Courts) + the affidavit written into the statute
Illinois Courts — approved forms — the official source for this state’s forms, free from the court.
- 755 ILCS 5/25-1(b)The small-estate affidavit's required contents are set out in the statute itself
- CCP 0248Cook County's small-estate affidavit form (circuit courts publish local versions of core filings)
Illinois is a split picture: the Supreme Court Commission on Access to Justice maintains statewide standardised forms, but the filings that actually run an estate — opening petitions, oath and bond, inventory, accounts — are mostly attorney-drafted, with circuit clerks posting local versions. Illinois Legal Aid Online's Easy Forms fill much of the practical gap.

Alabama
No statewide probate forms library exists
There is no statewide probate forms library to link to for Alabama — that is the honest position, and we would rather say so than link you somewhere unofficial. Start at your county probate office: petitions and inventories are obtained from the county probate judge’s office or drafted, because Alabama probate is run county by county.
- County sourcePetitions and inventories are obtained from your county probate office or drafted — Alabama probate is run by 67 elected county probate judges
- Worked exampleAutauga County publishes its probate fee schedule and the 60-day inventory rule on its official probate page (autaugacounty.alabamacourt.us)
This is the weakest forms position in the index, and we say so plainly: Alabama publishes no statewide probate forms library. The Administrative Office of Courts' e-forms cover other case types, not probate. Start with your county probate judge's office.
